What We Are Looking For

We are looking for a Part Time Nurse (Under 30 Hours/Week). To assist with our clients. Our in-home care Nurses assist with Medication Management, Wound Care, Client Assessments as well as personal cares required for each client. Ideal applicants will have a flexible schedule available in the morning and afternoon and located, preferably, in the Salt Lake City area.

Responsibilities
  • Be a client advocate as well as a positive role model for employees, co-workers, the community, and management
  • Develop, communicate, implement, and periodically update the clients’ Skilled Plan of Care (SPOC) and Medication Administration Record (MAR) with their Primary Care Physician
  • Assess the clients’ condition at every visit and communicate relevant or sudden changes with the clients’ family, Physician, and DSS management, as necessary
  • Perform intake visits as assigned under the direction of management
  • Obtain and document physician orders and changes to care
  • Update the clients’ Skilled Plan of Care and Medication Administration Record (MAR) every 50 days
  • Complete and submit a Nursing Care Log with every visit
  • Complete Certified Nursing Assistant (CNA) Supervision as assigned
  • Assist in development of Nursing policies, protocols, and procedures
Qualifications
  • RN Degree from an accredited School or College of Nursing
  • Ability to provide punctual, professional, and friendly interaction during client visits
  • Computer fluency, specifically with Microsoft Office, Word, and Excel
  • Must be highly organized and able to work without direct supervision
  • Must have good interpersonal communication skills
  • Fluency in oral and written English
  • Ability to lift 50 pounds preferred
  • Maintain current and unencumbered Utah RN license
  •  CPR/First Aid
  • Re-certifications
  • Staff meetings
  • Additional training per year as required or as scheduled
  • Required to pass a criminal background check
